HLCM051’s ARDS development is at the stage of progressing from a Japan-first clinical trial, following PMDA’s acceptance of the clinical trial notification, to a global Phase 3 trial and an application for conditional and time-limited approval. In FY2026-Q3, we will want to confirm whether actual patient enrollment and application preparations are proceeding as planned.

In the Japan-first REVIVE-ARDS trial, whether enrollment of the first patient has begun, and whether the start date and enrollment count will be disclosed
For the clinical trial notification accepted by PMDA in February 2026, whether progress will be disclosed on trial initiation, the number of sites and patient enrollment
Whether the filing timing for conditional and time-limited approval, outstanding issues in consultations with PMDA and the relationship with the post-approval confirmatory trial will be specified
Whether there has been any change to the plan for the 500L 3D bioreactor at the Kobe manufacturing site, annual supply for 40,000 people and completion in January 2028
The company's ongoing themes and initiatives this watchpoint touches on.
HLCM051’s ARDS development is shifting to a structure in which the REVIVE-ARDS trial will serve as the confirmatory trial, based on the results of the ONE-BRIDGE trial completed in Japan and the MUST-ARDS trial conducted in the US and UK. In FY2025, the company submitted the clinical trial notification for the Japan-first trial to PMDA and explained that preparations to begin the trial had been completed following a 14-day review period. In FY2026-H1, it disclosed that PMDA had accepted the notification in February 2026. The important change is the shift from the previous policy of pursuing a standalone domestic Phase 3 trial to a policy of using a global Phase 3 trial to seek conditional and time-limited approval in Japan.
At the next earnings release, it will be necessary to confirm the move from the procedural progress of acceptance of the notification to actual clinical execution. Specifically, we will want to confirm the date of enrollment of the first patient in Japan, the number of enrolled cases, the number of trial sites and how Japanese cases will be positioned within the global trial’s maximum total of 550 cases. Current disclosures do not show the enrollment start date, enrollment count, site-by-site progress or enrollment pace, so acceptance by PMDA alone cannot be taken to mean that clinical development risk has been eliminated.
On manufacturing, the company has outlined a plan to complete the Kobe manufacturing site in January 2028, install a 500L 3D bioreactor and build capacity to supply ARDS treatment to 40,000 people annually. Preparations for commercial production with Minaris Advanced Therapies are also under way, with the aim of reducing costs and achieving large-scale production through its proprietary 3D bioreactor manufacturing process. However, the amount of capital expenditure, the receipt and use of subsidies, the timing of process validation completion, the manufacturing cost per dose and the specific terms of commercial supply agreements have not been disclosed.
Accordingly, in FY2026-Q3 it will be important to confirm whether patient dosing began after PMDA accepted the clinical trial notification, whether the timing of the application for conditional and time-limited approval has been specified, and what outstanding clinical data, manufacturing process and quality-control issues remain for the application. In particular, the disclosed annual supply capacity of 40,000 people represents future manufacturing capacity and does not mean that approval has been obtained or sales have begun. The application timing, post-approval confirmatory trial and manufacturing-site start-up need to be tracked as separate milestones.
The focus after PMDA’s acceptance has shifted to whether domestic patient enrollment and the manufacturing and approval-application processes can be confirmed at the execution stage.
